We are committed to provide opportunities for generalization and CMS is pushing for “value based” therapy to a function but, how do we do it?
Here are the facts:
Being task specific in memory is the practice standard for more severe memory impairments.
In assessment, include PROMS (patient reported outcome measures) to gain insight from patient and family if possible.
Motivational interviewing can help with patient focus and understanding of goal of therapy. For example, if the patient wants to go back to work.
It may seem like a far stretch, but with motivational interviewing you can discuss the pros and cons find a task focuses on goal that is patient driven.
SMART (subjective, measurable, accurate, realistic, timing) goal planning with patient will lead you to your plan of care.
Your assessment will truly set you up for task management since he/she provided you with all the information needed to address memory.
For example, if working on spaced retrieval, you would tie in a basic work related task like knowing the office phone number or name of supervisor.
If working on external memory strategies, you may include work related information on “orientation page” of memory book.
Depending on the direction of your therapy and outcomes, the patient may not be able to end up returning to full work capacity (exact same job) but, the patient is motivated to learn and your initial set up may lead into more task specific situations that the same strategies could be applied.
